1. By signing herein, I/We confirm that the information given by me is true and correct. I understand that falsifying any information stated in the application and to the documents submitted to MACTAN BANK is a sufficient ground for legal action and for the rejection of my application. I/We understand that should my application be declined, MACTAN BANK has no obligation to furnish the reason for such rejection.

2. Upon acceptance of my/our application, I/we bind myself/ ourselves to the Terms and Conditions of all loan agreement as well as other relevant documents including, but not limited to, joint and several liability for all charges, fees and other obligations necessary in processing my/our loan application.

3. I/We authorize MACTAN BANK to conduct verification with the Bureau of Internal Revenue (BIR) in order to establish authenticity of my latest ITR and other submitted financial statements and hereby waive the confidentiality of any personal information obtained in the course of the verification. The information that may be obtained by MACTAN BANK will serve as basis in approving my loan application.

4. I/We hereby authorize MACTAN BANK to furnish information concerning my deposit accounts, placements or credit dealings and any other information that may be requested by other banks. I/We hereby expressly waive all statutory or regulatory provisions governing confidentiality of Bank transactions particularly, but not limited to Section 87-A of the General Banking Act as amended, as well as Republic Act No. 1405, R.A. 6426 and the Bank's compliance with Anti-Money Laundering Law.

5. I/We fully understand that, when information on my/our loan account is reported to any credit bureau, such information will become accessible to other members of the bureau, and may be used by them in connection with their credit investigation, analysis, and decision processes.

6. I/We further agree that the loan application shall be subject to Bangko Sentral ng Pilipinas’ rules, regulations and policies of the Bank and hereby undertake to comply with/submit all the loan requirements. I/We agree that the application may remain the bank's property whether the credit is granted or not.

7. I/We voluntarily agree that MACTAN BANK has the sole option to debit/credit any of my/our MACTAN BANK deposit account(s) to cover the payment in full any loan installment or payment due including but not limited to late payment charges or penalty, insurance, interests and charges provided in the loan agreements and such other documents executed that are relevant and connected therewith.

8. I/We authorize MACTAN BANK to send me/us updates about my/our loan and other product promotions/advertisements of the bank via SMS/text, email, mail or other means of communication.

DATA RETENTION

Records of your inquiries with us are kept only for a period of five (5) years afterward the said records are destroyed and disposed of.
